What do our assessments involve?
To maximise independence, we work in a person-centred manner
We work closely with clients to plan how they want support and to identify achievable goals
FACE TO FACE
HOME VISIT
Our home visits gather important client information including goals, preferences and care needs
RISK ASSESSMENT
During our home visists we also conduct a risk assessment, to ensure a safe environment
SUPPORT PLAN
We develop a support plan with the client and family members to clarifiy required support
SCHEDULE
Once a schedule is agreed, a selected team of carers will support your needs and goals
REVIEW
After 4-6 weeks, we review our service and support, via a home visit, phone or video call
